The CDH Research Survey went live on April 1st but in order to take full advantage of all research opportunities, we need ALL families to fill out the complete CDH Research Survey.  

New sections have been added as of May 18th so please make sure to log back in and complete everything!

The first 100 to finish the survey receive FREE CHERUBS HATS as our thank you to you for helping us to research the causes, prevention and best treatments of Congenital Diaphragmatic Hernia.

3. Fill out the FULL form, all pages

You are eligible to participate in the CDH Research Database if:

1. The patient is a survivor over 1-year-old
2. The patient is a non-survivor
3. You are a registered member of CHERUBS
4. You updated your information (has the patient since be born or passed away?)
